We accept most dental plans that allow you to choose your dentist. We are happy to file your claims for you. We encourage you to check with your insurance company before your appointment to understand what procedures your plan will cover. Membership Details:
- Patients with no dental insurance are eligible for membership.
- Smile Alliance Core Plan Details: Annual membership fee of $119.00 for individuals. Each additional family member is $79.00 for an annual membership fee.
- Smile Alliance Deluxe Plan Details: Annual membership fee of $299.00 for individuals. Each additional family member is $250.00 for an annual membership fee.
- These plans are not to be combined with any third-party financing.
Smile Alliance Core Plan Membership Benefits Include:
- 20% off two routine exams per year
- 20% off two routine cleanings per year
- 20% off one set of x-rays per year
- 20% off one application of fluoride per year
- 20% off two periodontal maintenance cleanings per year
- 20% off of dental implants
- 20% off orthodontic services
- 20% off all other procedures
Smile Alliance Deluxe Plan Membership Benefits Include:
- Two routine cleanings per year
- Two exams per year
- One set of x-rays per year
- One fluoride application per year
- 25% off two periodontal maintenance cleanings per year
- 25% off of dental implants
- 25% off of orthodontic services
- 25% off all other procedures
Terms and Conditions:
- Annual membership fee is paid at the start of the 12 month period.
- Membership fees are non-refundable and there are no rollovers into the next membership year.
- Payment in full is due at the time of the visit for all procedures.
Exclusions and Limitations:
- Services provided outside our office.
- Services not considered reasonably necessary or recommended.
- Any procedures covered by insurance or worker’s compensation.
- Services rendered before or after program effective or expiration date.
- No discount available for retail products or services (including whitening)

Payment Options
We accept VISA, Mastercard, American Express and Discover, personal check or cash. We offer a 5% bookkeeping courtesy if you make payment in full with cash or check at the time of service.
